With Flight Coverage Plus(sm), you can provide all or some of your Corporate Cardmembers with an even higher level of protection than what is automatically provided at no cost with Travel Accident coverage. This program provides your Corporate Cardmembers with air and ground accident insurance for up to 30 days, anywhere they go during their business trips. Flight Coverage Plus not only covers both domestic and international travel, it pays in addition to any other personal or organizational coverage that your Corporate Cardmembers currently hold.

Supplemental Air Travel Accident Protection
- Flight Coverage Plus covers a Corporate Cardmember while riding as a passenger
in any scheduled aircraft, as well as to and from the airport in a hotel/rental
company courtesy van or public conveyance such as a bus, taxi, or train
(private/rental cars excluded), and while on airport premises, provided the scheduled
airline ticket is charged in advance.
- Choose from principal sum amounts of up to $500,000 or $1,000,000
24-Hour Travel Accident Protection
- Corporate Cardmembers are also given ground protection that provides coverage
wherever the flight protection does not -- 24-Hour Travel Accident Insurance for up
to 30 days for any trip if they are on the ground and not en route to or from the
airport in a conveyance as stated above, or on airport premises.
- Coverage is for a principal sum amount of up to $150,000.
Organizational Coverage
- Your organization can receive up to $500 per ticket in Trip Cancellation,
Interruption and Airline Bankruptcy Insurance. If the trip must be canceled or
interrupted due to the Corporate Cardmember's personal or family illness or death,
or the airline cancels the flight due to bankruptcy, your organization could be
reimbursed up to $500 per ticket.**

Enrollment Instructions
Your organization may choose to enroll all or only selected Corporate Cardmembers in Flight Coverage Plus. To designate covered Corporate Cardmembers, you must designate the individual Account numbers of the Corporate Cardmembers. You must also notify your travel agencies so that they may update the selected Corporate Cardmembers' profiles. Each Corporate Cardmember's ticket must be billed separately in his/her
name to ensure proper coverage.

You may enroll Corporate Cardmembers in Flight Coverage Plus through paper enrollment forms, or through an on online enrollment form that the Corporate Cardmember must complete. To request enrollment forms, call (800) 627-4678. These must be signed by each Corporate Cardmember you wish to enroll.

Corporate Cardmembers may enroll online at http://www.dinersclubnorthamerica.com/US/en/diners_club.jhtml?pageId=us_03_01_04_06_04. You may provide this link to your Corporate Cardmembers via email, or own your own organization's intranet site. (If you choose to place a link on your intranet site, you should ensure that Corporate Cardmembers are aware of your organization's policies for participation and enrollment in Flight Coverage Plus).

The plan administrator, Citicorp Insurance Services Inc. (CISI) will enter Corporate Cardmember enrollments upon receipt, at which time enrollment will become effective. CISI will send a detailed Description of Coverage to each individual enrolled. Corporate Cardmembers can designate beneficiaries by completing the Beneficiary Form, which is included in the Description of Coverage.

Flight Coverage Plus coverage and fees will automatically be applied to any tickets purchased for enrolled Corporate Cardmembers with the exception of tickets included in packages booked by a travel agency. When this occurs, the Corporate Cardmember should notify Diners Club that an airline ticket was purchased to ensure they receive their coverage without delay. Your organization has the option of individually or centrally billing the coverage charges.

Legal/Claims Information
All claims are referred to CISI for compliance issues. When your Corporate Cardmembers, their families or their organization must file a claim, they should follow our procedures closely. A completed claim form must be returned within 90 days from the date of loss for the claim to be considered.
